The goal setting project will start on Monday, November 15 and will end on Monday, December 6, 2010. The project consists of three parts.
Part 1: Personal Evaluation and Goal
Complete the worksheets and create a healthy eating/fitness goal. You will work to accomplish this goal over the next three weeks (November 15- December 3). Make sure your goal is very specific. This is due on November 15, 2010.
Part 2: Tracking My Progress
Every day, you will log what you have done to reach your goal on the Tracking My Progress worksheet. I will be checking this worksheet once a week (counts as a quiz), so make sure you bring this worksheet to class every day. This worksheet is due on December 3, 2010.
Part 3: Assessment
You will answer a few questions at the end of the three weeks an will assess how you did in accomplishing your goal. The evaluation is due on December 6, 2010.

Violence and Injury Prevention Assessment
Complete ONE of the following as an assessment for the violence and injury prevention unit. This assessment is worth 50 points. It is due Wednesday, November 10, 2010.
1. Create a “Health Actions Pamphlet” to share what you have learned in this unit with others and help others make safer choices.

Guidelines:
Fold a piece of paper into thirds. Include the following in your pamphlet:
Page 1: Name, Period and Title of Pamphlet
Page 2: Paragraph description/introduction (what is the topic about and why is it important?)
Page 3: Prevention Strategies (how teens can prevent this from happening to them)
Page 4: Community Resources (places teens can go to for help- includes hotlines)
Page 5: Facts/Statistics (how common is this type of violence? Who does it affect?)
Page 6: Sources (where did you get your information?)
You will be graded based on the following:
Creativity and Neatness (5 points)
o Your pamphlet should be colorful, neat and organized
o Follow the outline given to you and make sure all pages are clearly labeled
- Paragraph Description/Introduction (5 points)
Should define and/or explain the topic and/or its importance
Facts/Statistics (10 points)
o Your facts must be accurate (two points per fact)
o Use the websites on the blog to find accurate information
Prevention Strategies (15 points)
o Your prevention strategies include practical things teenagers can do to reduce their risk of unintentional injury.
o Must be based on factual information.
Example: Prevention Strategies for Sexual Assault: Don’t drink: 50% of victims and 75% of perpetrators were under the influence of alcohol at the time of the sexual assault.
o Do NOT repeat facts
Community Resources (10 points)
o List three resources students can use for more help, counseling or assistance. This can include hotlines, websites, clinics or other resources.

*Research Danielle DiMedici and Jessica Tush. Briefly describe who these girls were and what happened to them. (10 points)
*Look up The Jessica Tush Act as well as Janele Hyer-Spencer's Domestic Violence Package. Explain what the purpose of these bills are and why they should be passed and enacted into effect by the New York State Senate. (10 points)
*Write a letter to a local politician (To whom it may concern:), explaining the importace of these bills. Use data and statistics, along with the stories of Jessica and Danielle, to support your justification for the passing of these bills. Make sure you include how important these bills are to young people. Use the websites below for more information. (30 points)
*Please have your letter reviewed and edited before submission. I will be mailing these!

3. Anti-Bullying, Anti-Stereotyping, Anti-Abuse Poster Campaign
Think about the topics we discussed of bullying, stereotyping, hate crimes and abuse. Use the insight you have gained to create a poster that visually depicts how much "Hate Hurts". These posters will be put on display during our school's Respect for All Campaign.
